A copy of the Region’s form to be completed by your agent or broker or insurer will be provided.

2. A Certificate of Clearance from the Workplace Safety & Insurance Board (WSIB) including your WSIB

account number.

3. Signed copy of Health and Safety Practice Form (to be provided).
